Sample Statement

Please note your statement may not list the financial section and/or the balance from prior completed terms section as you may not have financial aid and/or a balance from prior completed terms.

  1. Payment remittance - Please detach and include this portion with your payment. The mailing address is located in the upper right hand corner.   Below the mailing address are four boxes.  The account number box lists the student's identification number which is the student's account number.  Date due box lists the date the total balance is due.  Amount enclosed is blank.  Please write the amount you are enclosing in this box.  The total balance due box includes any prior balance from completed terms. 
  2. Charges and Credits - The date, description, charges, and any credits are calculated in this section.
     
  3. Financial Aid - A description of your financial aid is listed in this section.  The net amount may be different than the gross amount.  The federal government deducts a fee.  
         
  4. Summary - This section is a summary of your term charges less any term payments and credits less any financial aid this term.  
         
  5. Statement Date - This section lists date the statement was printed. 
         
  6. Date Due - This is the payment due date.  Your payment must be received in the student service center by this date. 
         
  7. Total Balance Due - This is the amount you need to pay.  
         
  8. Balance from prior completed terms - These are balances due from prior completed terms.  If there is no prior completed term balance, this section will not print on your statement.  Balances that are debit and/or credit balances will be included in the balance due box at the bottom of the statement and the balance due box in the payment remittance tear off portion at the top of the statement.  The current term's activity is not included as payment agreements are still in effect.  
         
  9. Schedule - Please check your class schedule.  If there are any questions or concerns, please contact your registration site or your adviser.

  10. Back of payment remittance - If you want to change the address to which you bills are sent, please complete the billing address change request section. This is for billing purposes only. Any other address change needs to be reported to the registrar's office. If you are paying by credit card, please complete the credit card information. Please remember to write in the amount to be charged on the Statement front in the amount enclosed box. 
         
  11. Terms and conditions - Information regarding due dates, payment posting information, late payment fee and payment options are listed. The student service center phone number and e-mail addresses is also listed.
